Is my graphics card broken?(suddenly black/white/green screen)

My desktop computer was bought 4 years ago.

Hardware Configurations:
CPU: Intel Core i5-3330 3.00GHz
GPU: Nvidia GeForce GTX 650 Ti Boost
MotherBoard: Asrock B75M-DGS R2.0
RAM: Adata DDR3 8GB
Power Supply: Antec 450W

I use DVI port instead of HDMI to connect my graphics card to the monitor.

I would say that my computer worked very well in the past 4 years. But something annoying me happened last week. When I was playing music in my computer, the monitor said no DVI signal and black screen occurred. But I knew that the computer was still operating since I could hear to the music. What I did was turning off the computer and rebooted it. However, I have tried this for several times. The computer could be turned on, but still the monitor received no signals. At that time, I thought that something may go wrong in my graphics card.

I have searched some methods in this website.
What I have tried to do:
1. Removed the graphics card, and connected the DVI wire to the display port of the MB. It turned out that the display works properly. I can see everything. Then I went to device manager and disabled all those display drivers. And I put the GPU back and restarted the pc. But it still did not work.

3. Bought a new DVI-D(dual) wire for connection: Useless

2. Cleared CMOS: Then, I opened the computer with my GPU installed. It worked on that day(also last for 2-3 days).

However just after a week, I tried to turn on my computer. My monitor obtained no signals again. I tried rebooting my pc. Originally, it worked. But When I was watching an online video(~ 720p) and using full screen. The screen became white and I decided to turn off the computer. A few minutes later, I turned it on again. Everything seems fine, but when I did the same thing(using full screen to watch video), the screen became green and the computer automatically restarted. I really dont know what is wrong with my graphics card. Could someone give me a help? Thanks so much!
