If the monitor is connected by the DVI to HDMI adapter, the problem is most likely from the adapter.
So what MB did you have? Also read the A/Q support from intel. https://www.intel.com/content/www/us/en/support/articles/000005556/graphics-drivers.html

Agreed, can you post the Motherboard name, and which connector is being used for which ones.
Also, try and swap around the cables, to check if all the monitors are okay.
