I have a solution for everyone. this combines all things i tried and things people suggested all in one spot.
Run a stress test like 3dmark or other program.
**If it crashes during test**
-Could be the overclock, turn off all overclocking
-Could be overheating, monitor temps to ensure adequate cooling
-Could be hardware issues, which is worst case scenario. Faullty power supplies are often the culprit, but could be GPU or motherboard. last resort is replacing hardware

**If stress tests passes without error/crashing**
-Most likely driver related issue. Either update to most current driver or if you have, try rolling back to an...

**If stress tests passes without error/crashing**
-Most likely driver related issue. Either update to most current driver or if you have, try rolling back to an older driver. (try installing in safe mode if needed)
-Try chaning both windows power to full or maximum performance, and in nvidia control panel if nvidia gpu. Set power to max perf.
-Try replacing your hdmi cable. Could be faulty or maybe a super long hdmi cable over 20' may be a problem.
-If after replaced hdmi cable doesnt work, try switching to DVI to VGA, or anything with an analog signal.
-IF trying a new cord doesnt work, try a different monitor, process of elimination here
-If screen still goes black after all this, try switching to windowed or bordless mode. (this worked for me the second time i had this problem)

One of these solutions should work for you. Worst case is replacing GPU. That should be last last last resort. I would test power supply if you can and replacing that is also a last resort. A bad motherboard is very very unlikely to be the issue but the problem shouldnt happen in gaming only. I have heard making sure the GPU is in the x16 slot can avoid issues, so you can try that too, but it shouldnt be a problem in x8. I play in x8 with no issues from that. Remember process of elimination is the name of the game here.
Good luck!
