Unusual PSU/MOBO issue causing PC to not completely shutdown

Just last night when I went to shutdown my PC something odd happened that's never happened before. The PC appears to shutdown normally (monitor goes black/turns off, keyboard and mouse turn off) however, the fans and LEDs kept running. The only way to completely halt the fans/LEDs was to put the power supply switch in the off position.

The specs of the pc are:
Asrock z77 extreme4 mobo
Corsair TX 650W PSU
Nvidia GTX 970
Intel i5 - 3570k CPU

I've tried a couple things by reading other forums and what not online. First I tried resetting the BIOS because apparently the BIOS may have had some sort of issue that kept the fans running. This did nothing, instead for whatever reason it caused the two USB 3.0 ports next to the PS/2 port on the motherboard to stop working. Then I checked all of the other ports because it was kind of concerning to all of a sudden have two ports stop working. Every other audio/USB/HDMI/DVI ports work other than those 2. Now, I'm confused as to what's going on.

The next thing I saw online was how windows 10 apparently has a weird issue with the way it handles power and that disabling the fast startup option may fix the issue. I tried this, and still no luck.

Something else that's incredibly odd is that when the power is turned back on (the switch on the PSU is put into the on position) the fans and LEDS automatically start going before I even touch the power button on the PC.

At this point I believe it is one of two problems: 1) the mobo is dying or is somehow requesting power from the PSU even though the power button was never initially hit. or 2) the PSU is going bad and isnt regulating the power throughout the pc properly. However, I've never come across an issue like this and am very confused. Any ideas on what is going on, or what part(s) I need to replace?

I just found the issue. The PSU was faulty and wasn't regulating power throughout the computer properly. I ended up going to a friends place and testing his PSU with my rig and the computer worked as it should: the power properly worked and completely shut down, and the USB ports started working again and I was able to successfully reinstall the drivers. I'm very appreciative for the help and different ideas that you guys had however! Thank you!
for USB 3.0: get into BIOS, advanced and have a look at the USB configuration. All there should be enabled, like USB controller, legacy,...

How did you reset the BIOS? Best thing to do would be to unplug the power plug from the wall, press the power button of your case for 5seconds and finally press the clrCMOS button next to the HDMI port.

Check inside windows, if an exclamation mark is shown in the device manager. Eventually update the drivers of the USB 3.0 controllers (Intel and ASMedia)

The computer shut down exactly like it did previously. Everything shut off, no display/mouse/keyboard however the fans kept running as it did before.

I reset the BIOS by booting into them and then resetting the settings to default. When I look at the BIOS everything is enabled. However, when I look at the device manager the host controller that I assume is supposed to be running those two ports is faded out without any exclamation point next to it.When I try to reinstall the driver I get an error saying that it can't find the host controller.

Alright, that re-enabled the host driver, however, now when trying to install the actual driver the computer blue screens with an error that says the thread exception not handled. Out of pure curiosity why did resetting the BIOS by using the clrCMOS button result differently than clearing the BIOS by doing it by booting into them? Does it not perform the same function?
 
eventually not all settings went to factory default when loading the default settings in BIOS, maybe the BIOS got twisted a bit.

Any devices plugged into the USB3.0 ASMedia ports?
Are you sure you are installing the correct driver? Try right clicking in device manager and "update driver" via windows

No, I didn't have anything connected to the USB 3.0 Ports. And the driver I tried to install was the driver from asrocks website, so I believe it would be the proper driver. Trying to update the driver via windows it says that it's already up to date. What's weird is that I had the computer powered down for the day and just turned it back on now and have the same issue as I had before I reset the BIOS using the clrCMOS button (the host driver is faded once again).
