Monitor loses signal randomly

I have been having this strange issue since I upgraded my graphics card to GTX 1050 Ti. Initially it would be limited to the game Witcher 3: The Wild Hunt only, when opening the inventory screen would result in the monitor turning off for a few seconds. Everything would return to normal after hitting escape to exit the inventory. However recently, this has grown to be a headache. This is no longer limited to the game but has started occurring randomly even when I'm browsing the web or launching other applications like EA Origin. Any help regarding this matter would be highly appreciated. Following are my system specs:
Processor - i7 4790K 4.0 GHz
Graphics Carrd - Gigabyte GTX 1050 Ti OC Edition 4GB
RAM - 12GB
Motherboard - Gigabyte H97M D3H
PSU - Cooler Master 500W
Monitor - Samsung S22E310 (1920x1080; 60Hz)

P.S:- I use an Active DVI-D-VGA convertor to connect my Graphics card to my monitor. I've tried using HDMI cable also, but there has ben no satisfying results.

don´t use the DVI-D VGA adapter, HDMI is way better

how hot is your CPU getting?

run ddu uninstaller, delete all previous graphics drivers and reinstall the latest nvidia.com drivers

try another monitor/TV
