EVGA GeForce GTX 970 Dual Monitor issues

I've recently did a rebuild on my computer, the graphics card I'm using is:

EVGA GeForce GTX 970 04G-P4-1972-KR 4GB SC GAMING, Silent Cooling Graphics Card

I've been running dual monitors, one to the DVI-I port and one to the DVI-D port, but one of them utilizes an adapter (truthfully I'm not sure which port, whichever port is wider is the one that's adapted), and I've had problems with some horizontal lines going through the screen while playing games. It's especially noticeable with darker screens, and while it's only a minor annoyance, it's not something I want happening on a brand new rebuild.

I have two questions. First, is the issue maybe that I am running an adapter to the screen that's playing the games? Has anybody experienced this before and could the problem be solved with a new cable to the monitor?

Second, I tried to use HDMI into the monitor to see if it would work, but evidently I cannot run one HDMI and one DVI cable at the same time. Is this accurate, or did I maybe do something wrong. If anybody has any advice it'd be much appreciated. Relevant (or not relevant) specs of the computer below:

Processor: intel i7-8700
Motherboard: ASRock Extreme 4
Ram: 16 GB @ 3000

I'm sorry, I mistook your thread to another one with SLI issues. You can use HDMI and DVI or DVI-D or DP at the same time, as long as your GPU has the slots for it.

So using one at a time doesn't fix it, the problem seems to be with the adapter to make it DVI-D. After a bunch of testing, the problem seems to be squarely on the shoulders of using the adapter, however I'm now a bit concerned if the HDMI won't work as the same time as the 2 DVI inputs. If I have it set as 1 DVI input and 1 HDMI input, it seems to work, but the 3rd input stops working. Is the motherboard output separate that i could run a 3rd screen (plan to run a wire to a TV in the room)?

No problem, things look to be sorted, and I'm mildly annoyed at myself that i didn't try running a game in the un-adapted screen to test it before. Thank you for your help!
