Is my GPU dead ?

so i have Gigabyte Geforce GTX 760 graphic card , i was opening overwatch like usual then i saw some small green lines scattered or over the screen , felt like something was really wrong with GPU then i got crashed and error said " rendering device lost " then when i rebooted my pc after windows load i get a black screen with white squares and lines then screen goes off into sleep and after i wake it up windows starts this also happens with every driver i install at the moment it activates the driver and when i go to device manager the GPU driver has a yellow exclamation mark , 0 VRAM in dxdiag and i have tried reinstalling and installing driver 234234 times different driver versions each time , SO does that mean my GPU is dead ?

Well, the problem then is either PSU or GPU.
Sorry to say that :(

well i just came back from a friend's and i tried my GPU there on his PC and it did same thing there too , black screen with white lines and squares , so does that officially mean the GPU is dead ? is there any solution for it like getting repaired or anything :S ?

Hmmm, there are no many possible causes except the ones you mentioned + low quality PSU which could damage it in some way.
Maybe simply GPU did hit its own life gap, i don't know how to explain it better.

mm i have high quality PSU i think , seasonic powersupplies are supposed to be :( and mm it's been 3 years 4 months since i bought the GPU isn't that too little for a life gap :(? , anyways my worry right now is this GPU died , so if i get a new GPU one the same setup , same MOBO, same PS ..etc is the new GPU gonna end up with the same result :< ?

Well, you should always be happy that GPU lasted for at least 3 years and we can in no way know if the next one will break that gap.
We can just hope :)
