So I have a 4 year old Medion PC(Art nr: 10019457) where I'd like to update the GPU of. The current one is a gtx745 and I wanted to update it to a gtx1050 but I was worried if it'd fit on my Motherboard(H81H3-EM2) and if it'd go well with my 350W PSU

Solution
The minimum recommended watts for gtx 1050 and 1050ti is 300watts, I would recommend going for gtx1050ti if you were to buy a gpu.
But even though is has the minimum watts there is no guarantee it would run it, you would need to know how much amps it delivers, do you know the exact model? Since your pc is prebuilt they have cheaped out on the ram, motherboard and psu, using the parts that can only just run the cpu and gpu they are paired with. I would recommend upgrading motherboard, ram, psu as well as gpu. What cpu do you have and what is your budget?

His motherboard may be able to support it but also need to check if psu will be able to run it aswell, could you take a picture of the numbers on the psu?

I5-4460 I was going for the 1050 considering it was on sale and considerably cheap. Preferably around the €100. And I have 12GB ram. And about to open up Pc to check the PSU again

For the second image, would the GTX be able to fit in there? The 745 is quite a small card

Your motherboard may support it, but your motherboard is not the only component to consider as abhishekuikey0907 thinks. You need to also make sure the psu will. I have seen the picture of your psu but I am not sure if this psu will, I don't want to give you false info so I will ask someone more experienced and knowledgeable.

Wait for a reply from me.

Thx a lot! I'll get an aircan to clean it out and the gtx, slightly concerned if it'll fit but I'll find out
