Okay all of a sudden my screen turned into this :

I installed a new windows but the computer freezes as soon as I log in and these colors appear as soon as I boot my computer even before the windows tries to start. So I wanna make sure what the problem is before replacing something. I have a powercolor X1950 Which is a pretty outdated card along with intel core 2 duo e6750 2.66. I took everything apart and reassembled components again but had no luck. So what exactly is broken and has to be replaced? Is it 100% my graphics card or something else could be causing this problem?
