Weird display/screen distortion?

I'm experiencing weird screen distortion after hard reboot. Screen wen't black with monitor reporting so I did a hard reboot with the power button, upon reboot I'm experiencing strange distortion. I tried taking a screen shot of the distortion, but it don't appear in the screen shot. OK for starters my screen/display is fine when I'm playing a game(only tested world of tanks) or when I have just a browser maximized, but soon as I click on windows start button/icon to bring up classic shell menu, I noticed tiny bottom right corner of my screen has a distorted rectangle, the distorted rectangle is almost like my desktop background and icons where a really tiny image that is repeated and distorted. I'll also get distortion when I have no windows maximized and I move my mouse or move a un-maximized windows(distortion is like tiny squares made up of my desktop all over my screen). I also get distortion when opening GPU-z, the blue gpu-z load square displays fine, but everything around it is distorted as if my desktop background image is a bunch of distorted squares. Also another weird thing I noticed is when I go to screen resolution(where you change scree resolution and detect displays) their are two monitors listed(monitor 1 and 2) when I only have one monitor connected. Both monitor 1 and 2 list the same monitor(which is the monitor I currently have connected), under monitor 1 properties it list adapter type as "Microsoft Basic Display Driver" and monitor 2 properties list adapter type as Nvidia GeForce GTX 750 TI" Their is no option to disable monitor 1 or change it resolution as it's grayed out, but I am able to change the resolution for monitor 2.
