Computer stopped detecting NVIDIA GTX 760.

I have been having a lot of problems with my computer recently where I will be playing a game and the computer will freeze up (screen freezes or turns black and I cant move my mouse). Usually it stays frozen for about 30 seconds but sometimes it requires me to press the restart button and fully restart my computer. This got out of hand recently when it did this 5 times in one night. I went in the BIOS to reduce my CPU overclock (I thought the CPU might be the problem) from 4 ghz to the default 3.4. When I restarted my PC my monitors (I have 2) were not working despite me changing nothing. I unplugged one of them from my graphics card (i normally have them both attached to the gpu) and put it in the port for intel integrated graphics on the mobo. The computer then worked but when I went to the device manager no graphics cards came up. When I showed hidden devices it showed the card as not being plugged in despite the fan on the card spinning. I have had no problems with overheating (I sometimes pull up open hardware monitor on my second monitor to take a look at temps) and I tried reseating the gpu and the ram with no luck. Is it time to get a new card? Is the freezing I saw a problem with the cpu or the gpu?

I built my computer 2 years ago and these are my specs:
Intel i5 3570K @ 4.0 ghz
NVIDIA Geforce GTX 760 (no overclock)
ASRock z75 pro mobo
8 gb ddr3 ram
cooler master CPU cooler
 
Solution
I would start by resetting your cmos, pull the battery from motherboard after you unplug pc from power of wall, let it sit for 3-5 mins, re-insert the battery back in place, Leave the GPU video card out of pc, and connect 1 monitor to your main board video connection and boot her up.
