My entire OS just randomly freezes during gaming

When I play for example Cities: Skylines for about 30 minutes my entire computer just freezes. I can move my cursor, but keyboard is not responding to any buttons and all windows are just frozen. It may sound like overheating, but both CPU and GPU are roughly at 60°C.

But the weirdest thing is when i play for example Tom Clancy's Division and my PC freezes again, I still can hear my teammates talking in voice chat, but they can't hear me.

My specs:
CPU: AMD FX-8350
GPU: AMD Radeon HD 7800 Series
RAM: 8GB DDR3 (by the way i checked the memory with memtest86 and there were no errors)

Solution
If those voltage reading are accurate, the PSU isn't able maintain correct tolerances. The _12V and +3.3v are especially bad and below the +/- 5% required. The rest are not very good either. So, the PSU is the first culprit I'd consider if it is a hardware problem. (and I think that 3vsb is suppose to be +5V SB instead)
You may want to verify those readings with an accurate digital VOM meter.

If so, the +12V rail can no way handle a HD-7850 and FX-8350 safely.

Do you have a lot of stuff running in the background when you game? Might be some poorly written code conflicting with the game.
Speaking of poorly written code, have you checked for Malware lately?
Lastly, run CCleaner to get rid of software conflicts and registry errors that may be causing issues.

Btw, what is the make/model of the PSU you're using? And which HD 7800 series card are you using?

Yeah, I should've post more detailed info about my PC -.-

PSU: Linkworld LW6-600 600Wt, 120mm
GPU: Sapphire HD7850 2GB
Motherboard: ASUS M5A97 R 2.0
I got two 1TB disks also checked and they don't have any issues.

No, I don't think this a software issue, because I've got fresh Windows installation. I fixed registry errors, but it didn't
help.

I'm actually concerned about my PSU, because I've got weird voltages. Not sure that they are normal.

I locked two cores from BIOS so now the CPU acts like 6-core processor and system work stable now. Maybe PSU doesn't have enough power?

Okay I guess a have some kind of a progress -.-
So I updated BIOS and now instead of completely freezing my PC just freezes for about 10-15 seconds and then unfreezes. This only happened once but I guess it solved the problem more or less. But I still very concerned about my PSU, because I got almost same voltages in BIOS and on screenshot I posted. Should I get a new PSU?

Yup, it is my PSU.
I'm pretty sure that those reading are accurate. Even if they're not I need a new PSU anyway.
I'm thinking about buying THERMALTAKE LT-650P. Do you think it will fit to my current build?

Fit your build? That will depend on the case (chassis) in which it has to go. But the Thermaltake is a standard ATX PSU, and that is what your old one appears to be also. So if the old one fits, the new one should as well.
As to connection match-up, all is good with the Thermaltake.
