gpu core and memory clock maxing out

hi all i seem to be having an issue with my gtx 780 my frame rates will drop to 1 and then it takes ages to close it or i have to hard reset the pc by the reset button my GPU clock and memory clock seem to max out and the GPU load and memory controller load spike it seems to do it under heavy load also it becomes very unstable when even being overclocked and underclocks its self with even the slightest change to the voltage at first i thought it was the power supply due to it only having 600w so i upgraded to a 1000w and the problem still persist anything over the stock speeds and it gets very unstable so it's kept at stock speed it's always been unstable in the overclocking area but no issues with it and the stock speeds until now
 
Overclocking isn't guaranteed. If it works at stock speeds "the problem still persist anything over the stock speeds" then there is no problem with the card. You mention voltage in this post but not temperatures. So my first step would be to see if the problems correlate to temps going up to a certain level.

your card is failing never ever over clock a card unless you got plenty of air in your tower i keep the cover off of mine so that heat can escape and use a box fan to blow out the heat.
if this step fails its ur card uninstall your card software and driver reboot and do a clean install pray that the settings was screwd if this works do not change anything leave it,use gpuz to see if it still persist.if so you need a new gpu

hi im on a water cooled system and my temperature under load can go up to 48 degrees or above when under a normal load it seems to be fine but when i play certain high end games i will get dragging textures the frame rate drops used to happen very rarely but seems to be happening more now.
