My Graphics card won't send a signal to my monitor.

My video card has been having trouble for a few months now. Whenever I would play games like For Honor, Dark Souls 3, or Battlefront 2, my video card would freeze, leading to my monitor having vertical lines across it and I would have to manually reset my PC with the power button.

No I can't even get a signal through it to my monitor with my HDMI cable. On board graphics connecting to the motherboard works if I remove the video card from the PCIE slot.

When it is plugged into the PCIE and connected to the power supply, it lights up and the fans spin, but no signal.

I checked the UEFI and the mobo said the PCIE was empty, but this could be since I didn't plug in the power cord to the card. However I can't open the UEFI with the card plugged into mobo with power since there's no signal.

I've had this PC build since December 2014, so I have had a few years of solid game use without major issues till now.
