I’ve got a Good Gaming PC built In 2012-13 It still can run High-End Games Like COD WW2 ,Watch dogs 2,Rise of Tomb raider and more in High or medium Graphics quality In 1080p it lags/Glitches sometimes and heat a Lot while Gaming. Specs of My PC are AMD radeon HD7800 2GB graphic card,Corsair vengance 8GB ram ,128GB SSD ,3TB,HDD,asus CROSSHAIR IV FORMULA Motherboard,i’ve got a new Corsair PSU 600W(CX600).My question is what should i upgrade for part time 4K gaming and less Heating and Glitches in Mid-Budget?

I can't see a mention of what CPU you have, though that has lesser effect for 4K gaming if relatively decent.

Graphics card wise you're looking at tremendously silly prices for what you'd need (at time of writing), as a GTX 1080 is generally seen as a necessity at those resolutions.

Personally I would try and sort out the heating issue you mention before proceeding to new hardware.
