SD Card Disconnecting when formatting and deleting files

I have a SanDisk 32GB SD Card, it is attached to my android phone, but suddenly it says "SD Card Damaged" tried formatting it through phone but it fails. Then I tried to connect it to the computer using CARD READER, it reads all of the contents, I can also copy files from it but whenever I am trying to delete files or formatting it, I notice that it suddenly disconnects to the computer and reconnecting again, tried DISKPART - Clean and Format but still it disconnects and reconnects again.

What seems to be the problem with my SD CARD?

It's damaged, probably corrupt memory chips - - bin it & by a new one (a no brainer this is, I mean they don't cost hundreds of pounds do they, so why bother with trying to fix it?).

I always replace my memory cards straight away when they start acting strange - - - a card you can't trust is only fit for one thing.
