My hard disk is making continuous sounds

My harddisk is making continuous sound while i am working on it it aso takes a lot of time to run or even open a file. Please help me resolve this issue.

Sounds like it needs to be defragmented.

Or you have to delete some junk off the drive.

Or, more likely, a combination of the above.
