I'm looking to upgrade my motherboard and CPU to be able to run FFXV. I'm hoping I don't need to upgrade the rest of the hardware:

Grab a ryzen 5 1600, some ram and a OC'ing motherboard:

But the thing is, that would be a horribly unbalanced system. I would suggest upgrading you monitor, cpu, motherboard and ram, along with getting an ssd, which can be done...

Do you want to keep you DDR3 ram? If so you have to stick to intel 6th gen processors or keep the motherboard and buy a AMD FX9590 processor.
Otherwise I would recommend selling your DDR3 memory and invest into something like
i5 8400 + intel z370 mobo + 8GB DDR4 RAM
or
Ryzen 5 1600 + AM4 budget mobo + 8GB DDR4 RAM

After this I would suggest upgrading your GPU to a GTX 1050 ti, perhaps, or getting a used r9 290 or GTX 970.
