So every time when i try to overclock my x4 955 to 4GHZ it just blue screens when i try to play GTA 5 or do stress test with Passmark Benchmark.
What i tried is multiplier 20x200 so it gets to 4GHZ and 1.4750 Vcore and still doesn't work.
Also when i try to overclock it with 16x250 and 1.4750 Vcore it also blue screens.
I left everything at stock now currently.

Here are my specs:
X4 955
GTX 660 OEM
10GB DDR2 800 MHZ
500GB HDD 5.4K 2.5 Inch
500W powerupply.
GA-MA785GM-US2H motherboard.

Solution
Limits of the chip. Not every chip is made the same and the quality of the silicon and variability in the other components affect overclocking. You probably just got unlucky.

Back down on the voltage and settle for 3825, the difference between 3.8 and 4GHz is barely noticeable and the tradeoff for it is not that worth it.

Thank you for answering me.
However at Passmark Benchmark when it is at 3825MHZ it gives me a score of 4980 of the CPU.
I tried it at 4GHZ and it gave me a score of 5368 which it is a huge jump of performance but when i try to start GTA 5 it bluescreens, only GTA 5 is giving problems.
Also at Cinebench R15 it gives me at 3825mhz 378CB and at 4GHZ 427CB.

at 4GHZ idle it is at 32 celcius degrees and at load it is hitting max 47 celcius degrees.
I have Hyper 212 Plus with 3 Fans.
What I have noticed is that the phenom x4 chips work quite cold even at load.
