Is my CPU failing, my HDD or is it something else?

I've noticed that for a month or so now, my computer has be acting rather slow in general. At first I was thinking that my CPU (i5 6400) was messed up because GTA V wouldn't run all that well and sometimes textures and objects wouldn't load, but then I was thinking maybe it's my HDD because my CPU I bought new, unlike my HDD which is refurbished. The weird thing is that even google chrome and loading tabs would be annoying so I was thinking maybe it's my RAM (Avexir Core Series (1x8gb)). I do play rocket league and CS:GO and they are fairly easy games to run, but it I still noticed a tiny bit of lag or frame skipping. I know that I don't have the best internet, but I doubt that my internet would make textures not load. Also when I get into Fortnite (pls no hate...) now sometimes buildings won't load properly for a bit and everything looks like blurry and broken. If anyone has any idea on what's going on please provide a suggestion.

My PC Specs:
Case: Phanteks p400s (the older acrylic model)
Fans: Came 2 came with, added an extra cheap one from my old computer
MOBO: MSI Bazooka B150M Plus
CPU: Intel i5 6400 (w/stock fan/cooler)
PSU: Rosewill Glacier 600w
HDD: 2TB Hitachi Ultrastar (Refurbished or at least according to the seller)
RAM: Avexir Core Series (1x8gb)
GPU: RX 480 Sapphire Nitro + (8GB + (I slightly OCed it))
Date build: Late 2015 or early 2016 if I remember correctly.

Get an SSD. Even a 128 gig will do if you can't afford a bigger one. Just use that one for system and base applications. You can keep games on the Hitachi as the loading times are only minimally affected, with most being run in RAM.
If you're using Chrome, it's very memory greedy, try and see if you're hitting your memory limits. GTA V and Chrome can easily burn through 8 gigs of memory. If you're experiencing stuttering, it could simply be windows trying to offload memory in to your page file on disk to try and free up memory for demanding applications.
But trust me on this, get an SSD for Windows and it'll be a huge difference.

I got the drive a while ago from amazon so I don't remember what the seller was. I don't know if this info helps, but I've only used 800GB of the 2TB. Also are you sure it's the hard drive, if your pretty sure that it is I might just try to sell or get rid of my current one and just buy a hybrid drive or shell out on an SSD because I don't want to deal with this shit anymore. Thanks for the help though :D

I don't have the "Hitachi Test Tool", but when I got the drive I got HDDlife installed with it. This is what showed up from it:

I make sure that the only thing I have open when I play video games is Discord and sometimes the Xbox app if one of my Xbox friends messages me.

If it doesn't work it's time to start examining other things.

Then again, I just noticed you OC'ed your GPU. Maybe try removing that to stock and see how that runs?

I was having the same issue with my PC when playing PUBG (buildings didnt load), it only happend to me when i was playing from my HDD, after reinstaling it to my new SSD it went all well. And maybe bc you are running 8GB Ram not in Dual Channel Mode.

Okay, When I get the time and money I'll try to get an SSD. Also should I just get a small capacity SSD for the windows software or should I get a an insane one (I don't have that much money, but if it means I can play my games and get work done without hassle than I'm pretty sure I can find a way). And I'll try to set the GPU to stock settings.

What do you mean by dual channel mode? I have only one RAM stick with 8gb of memory. I'm not sure if I'm misunderstanding or if you didn't know that I have 1x8gb set up.

128 gigs is enough for your needs. You don't really need a fast drive for most games, you'll only notice a bit longer loading times usually. But for OS it'll make a huge overall difference.
If you can't afford to go bigger, 128 will do. No need to go over budget if you can't do it.

I saw it at you PC spec above, that means you have only one stick in your board. Its better to have 2x4GB than1x8 you can possibly get more fps.

And if you buy an ssd dont go for the expensive ones buy just a cheap one there is no difference in speed (i just bought the new Samsung 860 EVO 250GB bc my old kingston v300 60GB was too smal and to be honest it is maybe 1 or 2 sek faster than my 4 year old ssd).
