Windows won't boot up when gpu connected..

Guys I'm having an outdated gtx 660..
A few days back..my screen turns black in middle game and when booty the display black... And other works fine and same thing happened again...
And now my pc won't boot when gpu connected
It's just did a half sec fan spin and turns off..
I don't know is tat the card dead or Psu problem.

CPU spec
Fx 8350 @4.0ghz
8gb ram
Gtx 660 2gb synergic edition
M5a99fx mobo
 
Solution
Best way to find out is to swap your GPU with another known working GPU.
- If the other GPU works, you will have a GPU hardware failure or a driver issue (although less likely if it happened mid-game)
- Have you ever checked what temperatures you have been hitting whilst under load?

Then if there is no luck, replace the PSU with a known working one that supplies the adequate power.
