Here is my card: CLICK ME :)

Here are some of my other components:
OS: Win10 Pro 64
CPU: Intel i7 3770K @ 4.2
Cooler: Corsiar H100i v2
MB: ASUS P8Z77-V Deluxe
RAM: 32GB Corsair Vengeance 1600MHz
PSU: Rosewill Lightening series 800W
Sound Card: ASUS STX Essence II
SSD: Intel 250GB for OS
HDDs: 1: 3TB WD 7200rpm for games 2: 1TB WD 7200rpm for media and backups.

I'm not sure what's going on. No matter what fan controlling software I use, when my temps get around 60C-65C, my fans go HAM. Like, 4300rpm loud af level. I've used both EVGA Precision OCX and MSI Afterburner, both have the same issue. The only semi-success I had was using Precision OSX as it would at least keep the fan constant where I wanted, but again once it reaches 60C -65C it goes 100% on fans.

When it does decide to ignore the controls set in place it can take about a minute for it to go back to my settings, if it does at all. Most times it'll just flucuate between 10% or 100% and never reach equilibrium.

I thought maybe it was just getting a heavy load since I am using an older processor, an Intel i7 3770k. So I OCed it from 3.5 to 4.2 successfully and stress tested. Everything runs perfect. Load up a game and the same crap happens again.

Mafia III won't make it go AS crazy, but all bets are off when it comes to Dirt Rally. When I boot Dirt I might as well be in the car itself because that's how loud this card gets.

This made me certain it's either the card or some bogus software I don't know about overriding what I'm trying to tell the card.
The only success I've had to make sure it doesn't go into maximum overdrive is if I make sure it never reaches 60C to begin with, which is hard with some games.

What do you guys think?

Solution
After contacting EVGA and explaining the same things the tech let me know to try swapping out my power supply as to them it seemed as If the voltage was too high or too low. Tested my PSU and sure enough one of the 12V rails was about .6~ over. Got a new 850W PSU and everything works perfect now.

TLDR: Changed PSU. Fixed problem.

Exactly. No matter if I use a custom curve, the one built in, or set a static speed. If temps hit 60C it goes straight to 100.

Maybe, I'll have to try and update/flash it when I get back home from work. If that doesn't work then ¯\_(ツ)_/¯

Yeah it sounds like only a replacement would fix your issue. It might also be possible that the card is designed to ramp the fans up to 100% at 60c and be uncontrollable but that would be a question for the manufacturer.
