Anyone can verify if one of my hardware components is failing? Help please

CPU is i5-4460 3.2GHz
GPU is: Gigabyte Geforce GTX 960 2GB GDDR5 G1 Gaming

Is my GPU failing? I have all the drivers updated for all components.
Also my BIOS is up to date.

Numbers on GPU are odd...according to Open Hardware Monitor.

FPS drops are floating quite alot, when I'm in spectating mode in Dota 2. It's FPS is like 20-25-30 but never a stable 60 FPS.
On League it's same story and, main menu screen at Total Warhammer 2 is outrageous bad. It's floating between 5-10 FPS and
Cutscene, loading screen takes forever.
