Crashing daily...tried almost everything...need advice

Okay, so this is a bit long winded but I want to give as much detail as I can in order to figure out what is going on here.

What's happening:
While under load (game running along with a couple background apps like discord and Twitch) my computer will freeze. When I say freeze, what happens is the screens go black like I put the computer to sleep, but the computer is still powered. Discord cuts out, and a few times the screen didn't go black and instead was just completely frozen. I think one time I got some windows screen saying something like "windows encountered an error and had to restart". 9/10 times this has happened however, it has just been the monitors going dark and the computer being unresponsive.

My Hardware/OS:
OS: Windows 10 Pro 64 Installed on a 120GB Corsair Force LS SSD
CPU: Intel Core i5 4690K Devils Canyon (No overclocking)
RAM: 16 GB Crucial DDR3
MoBo: Asus Z97-A
GPU: Radeaon R9 380 4GB (No overclocking)
Case: Antec 300 Illusion Case
PSU: EVGA NEX650G

What I have done so far:

  • Wiped SSD and did a clean install of Windows

    Updated BIOS

    Cleaned case of dust and reorganized cables to maximize airflow in case

    Stress tested GPU and RAM, both seem fine.

    Monitred CPU temps up until the point of the crash. They did get a little hot and I have an order in for more thermal paste and a new CPU cooler (I'm using the stock Intel one that came with my i5), but I also read that the critical shut down temp for my i5 is 77C, a full 10C hotter than they appeared to be at the time of the crash. (However at a previous point I used a different program which showed the temps pushing 90C, but that doesn't make much sense since the safety should have kicked in well before that?) Here are the last 30 seconds before the most recent crash recorded with RealTemp. I was running Fortnite, Discord, Steam, and Twitch (Intentionally trying to put a load on the CPU)

    Made sure my power management settings were set so that the computer shouldn't go to sleep unless I tell it to, and set it so that USB power is constant, etc.

    Checked all connections in the case, etc.

I have two theories:

One, it's either the CPU is getting too hot and those temps RealTemp is showing me are low or the sensors are malfunctioning. This makes sense due to the fact it probably needs new thermal paste and the stock heatsink/fan that came with the 4th gen i5 wasn't that robust. But temps don't seem to be that hot either.

Two, PSU is failing. However the PSU isn't that old really, I got it about 2 and a half years ago, and if I'm not putting a load on the CPU, it doesn't seem to crash. Crash only happens when gaming it seems.

Anyone want to lend weight to one of those theories or offer a different one?

use msi afterburner, then set the fans to an aggressive cooling curve, I set my rpm to match temp, 50c - 50% fan etc, then at 70c I set it for 100% (NEVER GETS THAT HOT) For your cpu fan I would set it at 100% and leave it there. The temps are going to shorten the life of your cpu at those temps. Make sure all the fans, cpu, gpu fans, and psu are clear of dust.

Ran Prime95 for 10 min (100% load on CPU) and got the following max temps:
Core 0: 82°C
Core 1: 83°C
Core 2: 78°C
Core 3: 74°C

Computer did not crash.

3x 120mm fans (2 in front, one in back), and a 140mm fan on top of case

Okay, on further testing, I think something might be up with the GPU. Just ran a performance test and the GPU gets put under a 100% load but the fans don't seem to start up much. GPU jumps to 100C and comp crashes. I tried again, just running a game instead and same thing happened except the game crashed instead of the whole computer.
