AMD Freezing and Black Screen while playing games

I've been having this problem for a really long time... When I play games such as League of Legends, Overwatch, Tera, or even Sims 4 they freeze then a black screen with no cursor pops up. The audio in game is still running, but I am unable to alt-tab or crtl + alt + del into task manager to quit the game. Everything will turn back to normal after the game crashes or I have to do a force shut down. This happens frequently, frustrating me every time.

My graphics drivers are up to date. Sometimes I get a "Graphics Card Error" message saying:
"There is a problem with your graphics card. Please ensure your card meets the minimum system requirements and that you have installed the latest drivers installed." I really wonder if the problem is just the graphics card, therefore, resulting in me getting a new card or something else.

I'm using Windows 10. 64 bit
Graphics Chipset: AMD Radeon R7 200 Series
Core Clock: 780 MHz
RAM: 8 GB
Memory Size: 2048 MB
Memory Clock: 800 MHz
Processor: AMD FX(tm)-4300 Quad-Core Processor, 3800 Mhz, 2 Core(s), 4 Logical
Motherboard: GA-78LMT-S2

if i had to guess, its a ram issue. i didn't even know you could get them clocked in the triple digits.

i had a slimier issue when i built my system, ryzen 7 2700x and it originally had around 2800 ddr4 ram. i was getting errors in every game i tried to play, about 5-10 im in, some just crashed to black screens and others gave out weird error messages along the lines of "Direct x broke". i was so confused and swapped out every part one at a time over the course of a month, leaving the ram and the hard drives alone because i thought to myself "theres no way these are the issues"... 3 GPU's, 2 x470 motherboards, and 2 Ryzen 7 2700x, upgraded PSU and nothing fixed it. once i gave up hope i just decided to slap in some better ram for the...
