Can't explain what just happend... - Samsung 960 EVO M.2 SSD

So I just ran through a peculiar situation that I'm hoping to get to the bottom of. I'm not sure if it's fixed, as of right now, as I'm doing additional "testing" on the drive to make sure everything's working properly.

For about an hour, or so, today I was running into an issue where my 500GB M.2 SSD was still being detected by Windows, but I couldn't access any of the files. Running the chkdsk prompt in the Admin PowerShell came back that the files were RAW and couldn't be accessed. I tried to reformat the drive so I could do a fresh partition but Windows couldn't run through the format process. I decided to restart the system to try and see if that would help - but when I booted it back up, it seemed as if nothing wrong was happening. The games I had on the M.2 were running fine. I tried installing something and after that was finished, the problem came back again, prompting another reboot.

I'd just ran anti-viral software and so far I haven't run into any additional issues; the drive is running normally, now, but I'm still not sure the problem's been fixed. Is there an actual explanation as to what's happening?

What information am I going to need to provide?
