GPU loses connection/crashes at 100%

I have a GTX 630 4GB. For the last couple of days, I had been able to get about 30fps from GTA V on this card but after a random GFX_INIT_ERROR, the card has begun malfunctioning. The display driver randomly crashes. Whenever I start GTA V, it crashes on the first playable frame. Same thing happens when I try the Witcher 2. On trying to update the GPU driver, Nvidia's installer can't install the driver (Have tried both manual and Geforce Experience) even though GPU's display driver is working fine at that time.

I know this is a pretty crap card for this day and age but I want to know if this card is dead? or if there is some way to fix it?

Hopefully its not a physical issue with the card.

You can try running DDU to completely and cleanly remove all display drivers then reinstall the most up-to-date version again and see if that fixes the issue.

If it keeps happening I'd download HWMonitor and Furmark and perform a benchmark and monitor temps. Does it crash on the benchmark?

Gotta rule out if issue is with game directly or if its with the GPU.

I removed all previous drivers using DDU but now it won't even show up in the device manager (it previously showed under display adapters but now there is only Intel Graphics). Trying to install new drivers also fails as before.
 
This sounds like an issue I ran into in the past.

Can you check control panel and uninstall anything that says "display adapter" in it. Nvidia has an issue with 3rd party display adapters that wont allow for newer drives to properly be installed because of it.

Also is the Intel Graphics drive installed and up-to-date as well?

Installed the drivers and it changed to GT 630 but new drivers still wouldn't install. Ran FurMark anyway and it crashed like before so definitely something with the card. The issue got more serious since the PC wouldn't show the desktop because of the card (Showed just a blank screen, all preceding boot up screens show up fine). Changed display port to Integrated graphics and boot up is fine. I guess I'll just have to live with the fact that this card is dead.
