Why do I keep getting screen tearing?

This is something that's been bothering me since a year now without finding any reason behind it but, since I got this monitor (Samsung SF350_S22F350FH) and switched from VGA to HDMI, I keep getting screen tearing in everything I do.. games, videos, even scrolling on websites and discord.. I don't know what the issue could be since on the old monitor which is like a decade old and VGA on this one I didn't have any issues. I have FastSync on since I noticed the screen tearing but I would like a solution to why it might happen. FastSync doesn't seem to work on all programs tho, my browser and OSU! being two of those.
GPU is a GTX 950 and the CPU is a core i5-7400

Edit: I decided to take a screenshot of the 3 kinds of tearing I get
1: A little rectangle in the top left corner of the screen/video

2: A horizontal line in the bottom half of the screen/video

3: A rectangle covering almost the entire left half of the screen/video

with "Fast response mode" you mean the "Key repeat time"option? because that one was set to acceleration and it's the only option that would make sense when it comes to faster response times

yeah, when I still used VGA I didn't have this issue. I haven't tested using VGA after that anymore because I don't have the adapter from DVI to VGA anymore since the GTX 950 doesn't have a VGA port (I really have to figure out where the adapter went)
