No Signal in-game

Hello I recently had a new cooler installed onto my GPU as it was overheating due to my last GPU cooler dying. Everything ran fine for 2 days until when in game for a good bit my monitor would lose signal forcing me to shut down my computer. If I attempt to manually restart I am met with the sound of 1 long beep and 3 short (no GPU detected) and no signal, if I shut down the computer then the monitor regains signal. This issue only happens in games and I have kept track of how much time I spend in games before losing signal.
Dead By Daylight: Last 15 minutes to up to 2+ hours
Minecraft: 15 minutes
Morrowind: A few hours
Rising Storm 2: 15 to 20 minutes
This is not an overheating issue as both my CPU and GPU barely even reach warm temperatures in game as I play open case. (I do clean my case out) It is also not the monitor as I have tested my PC on two separate monitors and used both AVI and HDMI cables.
My Specs are:
Processor: AMD FX-8350 Eight-Core Black Edition with after market cooler
GPU: Nvidia Geforce GTX 960 2GB with after market cooler
Motherboard: ASUS Crosshair V Formula Z
PSU: Apevia ATX-BT650W 650W
OS: Windows 7 Home Premium Service Pack 1
Memory: 8GB RAM
Sorry if my specs are not labeled correctly. I'm not the best at PCs or how to work them.

 
Solution
when your gpu fan failed it may have damaged the cards vrm. under load they may be failing. see with msi afterburner the voltage of the gpu is fine under game or stress load. if it not the gpu try a test power supply see if your low end unit is not holding when loaded. if it not the gpu or power supply check that your mb vrm are not runniing hot.

The paste was re-applied I would imagine (I did not install the new cooler) as the new cooler keeps the GPU at good temperatures. I'm checking if all my drivers are up to date at the moment and if it is not the solution I played on checking the paste or rearranging my GPU if loose.
