Z77 extreme 6 wont overclock

Hello, i have attempted to overclock my cpu i5 3577k motherbord z77 extrme6.
I have gatherd a lot of info on how to, forums, YouTube ect. But it just wont overclock.. i see in bios that the MHz is lets say 4200 but when i run stress test and monitor my cores they max at 3399 mhz. The only time it worked and i saw the cores go past 3399 MHz it was with intel extreme overclock tool.. But that crashed and i dont want to use that..
Hope some one can help

What stress tests are you using? Even if it were unstable, the clocks would go past 3400MHz under a stress test if you overclocked to 4200MHz.

Sounds like the Intel tool is stressing the CPU appropriately, but you're not stable.

What settings have you changed in the BIOS? What kind of voltage are you using?

Using prime and hwmonitor.
Settings
CPU Ratio = All Core
All Core = 42
Turbo = 0.004
Spread Spectrum = Auto
Internal PLL Overvoltage = Disabled
GT Overclocking Support = Disabled
Power Saving Mode = Disabled
CPU Voltage = Offset Mode
Offset = - 0.005
CPU Load Line Calibration = Level 5
C3 State Support = Disabled
C6 State Support = Disabled
