HDD always at 100% (tried multiple) while SSD at 0%

I have had this issue for a while now, about a month and a half. I have reset my windows, tried different sata and power cables, tried two different hard drives, but the issue still persists. I cannot play any games as my SSD cannot store more than one, and the hard drive crashes steam whenever i try downloading anything. When i go into task manager, it says the disk usage for the hard drive is at 100%, but it says both read and write are at 0 KB/s. I have been looking for a solution for a long time but cannot find anything that works

My specs are:

Ryzen7 1700 CPU
GTX 1060 6GB GPU
MSI B350 PC MATE MOBO
Two hard drives i have tried: 500GB from laptop, and 3TB Toshiba P300
My SSD is Sandisk SSD Plus 120GB
16GB TridentZ RBG 3000MHz
CX750M PSU

I have installed the latest Motherboard BIOS from the MSI website

Thanks for the reply. I have tried both of these. In HD Sentinel, it comes up with the stats and the health is at 0%. But it worked perfectly about 2 hours ago when i was copying files onto it, only when I tried to install GTA V through steam did it start having all these issues, and on the the other (Toshiba P300) hard drive, it just suddenly stopped working when i tried to update PUBG through steam

This is the second disk this had happned to, and the other was only one month old, and it only happens when i download steam games on it. There must be somthing causing this

This is what it says about the disk;

Replace hard disk immediately.

 
Do as it says, the disk is damaged.
Maybe you are mounting it wrong and its vibrating too much?
The screws must be tight and not loose and the HDD must not be moveable while installed.
The disk is really bad, and it should tell you how old the disk is as well as how many hours it's been used for.

I just copied GTA, an almost 80GB game over from another PC, in around 15 minutes onto the hard drive at around 80-100 Mb/s, then tried to tell steam where it was and it scanned the files super slow, at 0.5 MB/s again, with 100% utilisation. Im not sure what the issue is, but i have tried a multiple hard drives and all of them have the same issue, so im fairly sure a new drive will not solve this issue
