My laptop had been freezing and slowing, I checked windows task manager and disk was 100%.
I checked my disk with HDDScan – Free HDD Diagnostic Utility
I got these results

Can it be fixed?

Agreed. Your HDD is hot, has many pending errors, and the reallocation count is flagging for errors. The best bet is to back up everything you have before you can't. Grab an SSD, conduct a fresh install of Windows, and put all of your files back on.

It's a pain to go through but you're at a point where you can still save your data.

SSD drives are still expensive and since this is a laptop would most likely have a 1TB HDD i would assume the OP would want to keep the same storage size for it

SSD drive's will make any PC perform better but depending on how much you are willing to pay for one you may will lose out on a lot of storage space you used to have

I also agree with this. However, if you find yourself using only a fraction of the 1TB drive then you wouldn't even notice a difference with a smaller capacity SSD. I've upgraded many laptops to SSD's and most of them had 1TB drives. The owners were using maybe 100 to 300 GB. Most people that use laptops for every day tasks never use the drive to it's capacity. Just pick what works for you.
