GPU Fans going 100%?

Lately, I will be playing a game and my screen goes black and My gpu fan starts spinning as fast as possible. I am not sure what is wrong. I use a EVGA gtx 1060 3gb. It has been working fine for a few months then this started happening. I doubt it is a psu problem, I have a 500w psu and the recommended is 400w. Any fixes?

EDIT: I reconnected psu cables into gpu. So far it seems to be the fix. I am guessing that last time I reconnected them, It was not all the way. Any other input would be very helpful.

Yep, took out gpu and put cables back in. I used Msi Afterburn to up the fan speed. This helped, and so far has only happened once more.

thermal reporting/heatsink could be loose/paste could be bad any of these could be a problem as this is a relatively new card I would guess that it is some sort of software problem. is there another system you could test this on?

Unfortunately not. However I have noticed very high gpu thermals while gaming (gpu seems to be the problem anyway) Gpu temp has been a steady 130-150F while gaming. No idea how to cool this, besides time and upping gpu fan speed.

Those GPU temps are fine. Don't measure them in farenheit, it gives you a bad idea of how high they really are. 65C is pretty good.

Then any idea why my monitor goes completely black and the gpu fan speeds up? It happened again and my mobo told me a cpu failure. Only when I reinstalled the gpu did my pc boot back up again.
