Is my GPU dead?

My GPU has no fan spin or light up on initial boot, I get no video output. I've set the card in both slots and have tested on integrated graphics(worked). Is there anyway else i can test the card or is it dead?

Have you checked and re-seated the 2x6 pin power connection into the card? Most likely the connector isint making good connection to the input on the video card. Unseat the power cable and look at the end of the power cable that seats into the card, probably their is a pin that isint seated well on the connector.

Previously during games screen would go black and lose signal in game audio could still be heard this increased in frequency until yesterday when it wouldnt display from boot.I gave re plugged the GPU cables and changed pci slot no difference.
 
How old is your power supply? Yes, it's not uncommon for a PSU to work well enough for low powered integrated graphics but fail in supplying the necessary power for a discreet card, even one tiered much lower than yours.

I'd test it if you have access to, or can buy, a multimeter. They can often be found fairly cheap at home centers and places like Walmart or Harbor freight, if you are in the US. If not, I'm sure you can likely find one at a local retailer.

Booting up using the iGPU and then downloading HWinfo, installing it, and taking screenshots of the system voltages as listed in HWinfo, then posting them here, might be helpful too. You'll want to run something demanding like Furmark, Heaven benchmark or Prime95 WHILE you take the screenshots of the system voltages so we can see if the 3v, 5v and 12v values are even close to where they should be or if anything else looks "off". All sensors, up and down the HWinfo screen. Run "Sensors only" when you start HWinfo.
 
Yeah, I would test it. While that wasn't a bad unit, per se, when it was new, it was also not a really terrific one either. 3-5 years would be my expected lifespan for that unit in a gaming machine with a discreet card installed.

I'd check the PSU first, and if that turns out to be fine, then I'd look harder at the card itself. PSU is cheaper to replace than a new card or motherboard in any event, and as old as that one is if you buy a new card or motherboard, you'd probably also want to think about replacing the PSU with a newer high quality unit anyhow.
 
PSU looks ok except I see no sensor values for the +5v system voltage. It would probably be a good idea to test the PSU manually using a multimeter and the instructions I linked to a few posts back.

Have you tried this card in another system as was suggested by godzilla?

Do you have only a single case fan?
