Quake 3, at every resolution just about and just about any in-game setting, is really smooth sometimes and extremely choppy the majority of the time. Any ideas?
Half Life 2 - Won't load (always stops responding), which is weird because countr-strike source loads and plays just fine.. just don't know about buying Left 4 Dead yet if Half-Life 2 won't load.
HL Counter-Strike 1.6 - 1600x1200 run perfectly (it has a lot of tearing but thats nothing but a monitor/vsync issue to resolve).
World of Warcraft WOTLK - 1600x1200 resolution on highest settings plays smooth.
Tomb Raider Underworld Demo - 1680X1050 Highest settings plays beautifully.

Any chance you use a screen saver? I disabled mine years ago when a couple of my games would run fine and then, after a few minutes, would start displaying a low framerate. It didn't happen with all my games, so it drove me crazy until I figured out what was causing it.
And I suppose you could try uninstalling HL2, manually deleting any files that remain for the game, then reinstall it.
