I've been very happy with my logitech G5, and I know many other will recommend this.

G5, £30.
Switchable dpi upto 2000, with 3 settings. Useful for FPS switching from fast twitch to sniping.
2 extra buttons on the left thumb that are programmable for WoW, currently got both pally taunts on a mouseover macro binded to em....very useful

I've got a G5. It's OK, but the first one I got died within a week. Had to RMA. Seems to be a big problem with them from what I can tell.
Some things I don't like are.. I have trouble with double-clicking it for some reason.. dunno why. The thumb buttons are hard to reach the way I hold the mouse... meaning it is easier to program the buttons onto the keyboard instead if you want to do anything FPS with them. The mouse scroll-wheel click is stupid hard to push in compared to other mice I've used - its really annoying to me. The thing has a weight set and I think it feels almost to heavy without putting anything in it.
If I could do it over again id get one of the smaller razer mice. Especially for FPS.
I've got massive hands and struggle with most mice. neve had a problem with mine although i know what you mean about the middle click.

Got the G5 here (2 of them actually), very nice mouse, smooth, reliable, the resolution switching is fantastic. I liked the weights myself, they add to the "feel" of the mouse. For me the mouse was a perfect fit for my hand. My 6 year old son finds the mouse too big, so his rig has a smaller logitech mouse. The G5 is by far the best mouse Ive had.
It's not the best, but I use the mx518. I've been happy with it for a couple of years now. It is a great value mouse if your looking to spend no more than 50 dollars.
