(This is a non-commercial survey and results will be published in the public domain. I'm saving all the sites I post to and will try to post results as we compile them. It has been difficult getting respondents from the US & Canada because the major North American forums are highly resistant to marketing. This survey is for academic purposes only and has passed a Canadian university ethics review - including ensuring that it is posted on a Canadian server due to US privacy laws allowing the potential of exposure of anonymous respondents).
