What are the temps of the other components of your PC? (Idling and in-game)
I use HWmonitor to check all my components temps, found here

Those temps are quite high. Check if the card has got any sorts of dust piled up. Increase the fan speed as well by using tools like MSI Afterburner. You'll risk permanent damage if you keep it this way.

My 4870 was ALWAYS hot. I put one of these badboys on it (http://www.google.com/search?q=atctic+cooling+twin+trubo+4870&ie=utf-8&oe=utf-8&aq=t&rls=org.mozilla:en-US:official&client=firefox-a#q=arctic+cooling+twin+turbo+4870&hl=en&client=firefox-a&hs=WWD&sa=X&rls=org.mozilla:en-US:official&prmd=imvnsfd&source=univ&tbm=shop&tbo=u&ei=5aidTrQSgcSxAuKggfkJ&ved=0CEQQrQQ&bav=on.2,or.r_gc.r_pw.,cf.osb&fp=8a1feda00662d6e1&biw=977&bih=659) and never looked back.

+1; judging by the age of your card, i would assume it is dirty. You would be suprised how many people have this same issue
