I activated my old Audiovox 9000 last night online.
It wouldn't receive calls (calls rang 5-6 times and went to voicemail), and
when I tried to call out, James Earl Jones told me they couldn't verify
authentication.

Well that means the system knew what to program your phone with and you put
the right number in. The only other thing I can think of is you may have
needed to wait longer for it to work.

Dean,
It happened to me last week. All I wanted to do was change from NAM1 to
NAM2. The change was easy on their part, but it wouldn't work. The system
isn't foolproof. VZ technical dep't had to help me re-program my v60.
After entering the 24-digit intialization code, all was well again. They
were very helpful.

also- it's not always a nam switch that causes it-
sometimes a phone just need to be authenticated for whatever reason. usually
seems to be an older model phone- just call cust care, ask if they can help w/
an authentication issue- or ask them to txfr you to tech support, it will take
these guys about 5 seconds to fix you up. it's not your phone ( well, not
likely anyway)
