You most likely have integrated weak graphics and a laptop.
You wont be able to upgrade either being a laptop.

Your computer can't handle the game well.

Turn all your graphical settings down to as low as they will go and see how it performs. GTA IV is a very badly optimized game. For good performance you pretty much need a fast quad core CPU and a midrange desktop GPU. Only the really high end gaming laptops are really capable of playing this game.

As said above, get a better laptop, or build a gaming desktop if you want to play this game.

Did you update the game? If not update it to at least 1.0.4.0!

Isn't the GTX 560 a desktop card?
