*HELP!* Custom print size problem

I have when xp sp2 edition. I have a okidata ml420 printer that I print on
premade form. that being said:

when I used to have win98, I could hit the print button and the little
window would come up that comes up before you print (the one that you choose
the pinter and number of copies etc) from that window I could choose the
ml420 and under the advanced button I could choose a custom paper size
because the form I use is 8 /12 x 8 1/2.

NOW with win XP When I choose the printer and go to advanced there is no
custom paper size choice. I have tried different drivers from the
manufacturer and the windows driver but there is just no ooption for custom
paper size.

Does anyone hae any ideas how to make a paper size of 8 /12 x 8 1/2? Any
ideas would be greatly appreciated!
