Huge glitch in far cry 3

i can't play far cry 3 at all,it runs smoothly for about 1-5 minutes in the first mission and then that happens http://i.imgur.com/stHeD.jpeg when i exit this also continues in windows and i have to reboot to turn back to normal,basically screen is like cut in half and left part went to the right and the right to the left

i5 3570k 4.1
HD 6870 Sapphire 1000/1200
1440x900
8gb ram

(played farcry 3 before with c2d 8400 and 4gb ram with same graphic card and had no problems)
