If it runs out while you're under water you start losing health.
You can lengthen by diving underwater an staying under for a short time.

Update: No, that's not it. When I'm underwater, I have 3 bars...my
health, my air, and the other one...I wonder if it has something to do with
the money I owe the hippie freak.

Yet another update...the air bar is actually blue, my health red, and the
mystery bar is white...with my gamma cranked up, the blue one looks white

It shows how much armour you have when in combat.
If it runs out while you're fighting you start losing health.
You can lengthen it by doing the vigilante missions to level 12.
