Only happened once.

No recent hardware upgrades, no software installs, no OC.

Any ideas?

Causes of that driver ntoskrnl.exe are
Keyboard issue
Miscellaneous corruption
Corrupt boot.ini file.
Missing boot.ini file.
Missing or corrupt ntoskrnl.exe file.
Windows NT installed on a partition bigger then 7.8GB
Corrupted hard disk drive or severely corrupted Windows.

So if it doesn't happen again I would not worry about. Windows trys to fix itself after any error in startup.
