Well, I would offer this. Short of a hardware error or a local PC software corruption error, this is actually fairly uncommon these days (compared to the Win98/WinME/W2K/WinXP/WinVista days. Each new version of Windows has improved in this regard (I beta tested each of these and more as they were developed).
If there is something specific you are refrring to, I think we can focus on that. But, your original thesis is reall too broad to tackle, in general, and it technically isn't the case these days.
