Problem started a couple weeks ago. Device manager displays USB controllers
and root hubs as working properly, no problems. However ANY device plugged
in to any USB port comes up as Device Unknown and is dead. No response from
the device. PC recognizes when something is plugged in but it doesn't know
what. All hubs and controllers removed and reinstalled, same with drivers,
driver updates done, Etc. Devices with software reinstalled. Nothing
changes. Any ideas?

Are the USB Controllers you are using mounted on the Motherboard? If
yes, it could be a defective MB. Also, check
in the Bios. Any USB entries should be enabled.
