I have run into the problem described on
http://support.microsoft.com/default.aspx?scid=kb;en-
us;873401

Apparently the only way to obtain the fix is to go
through MS support channels to the tune of US $210.00 per
hour and hope that at the end of that process they are
willing to refund the money, seeing as how all I want to
do is obtain a known patch for a known problem.

You have got to be kidding!

I don't want a passport account. I don't want to pay $210
then have it refunded. I don't even want the
SF_NOTIFY_PREPROC_HEADERS notification functionality
removed from inetinfo after it has been working for how
many years?!?!?! I cannot believe Microsoft is getting
away with this! Ripping out features and then
withholding the patch?

For anyone else who runs into this problem and doesn't
want to pay the fee, here is your FREE answer: restore
the w3svc.dll and wam.dll in the system32 folder from
backup, and you will be able to continue using your ISAPI
filters with IIS 4 on NT4.

When you call MS for hotfixes you just have to tell the customer service
people your after a hotfix and the KB article and they won't even ask for a
credit card. The CS people will send you the hotfix. Also it is not $210/hr!
Last time I had support it was $250 per incident. So that could be 1hr or 20
hrs. MS is not withholding anything, they do not release these fixes because
they are not tested but typically will fix your issue.
