could this be a graphics card malfunction?

My friends been having his monitor display a black screen with a horizontal and vertical line cutting the display into four equal spaces. we tried putting in a new hard drive thinking that may have been the problem (maybe it had crashed) but now are wondering if it may've been the graphics card?

If anyone has any clue or help for us that'd be awesome.

That is definitely the type of thing a dying or dead graphics card could do
