Accounts Locking Out For No Known Reason

I have 3 servers on 3 separate sites, each with Windows 2000 SP4. Each
server is also running Exchange Server 2000. For no apparent reason some,
sometimes all, sometimes a few, accounts get locked out. This happens on a
daily basis, and even happens on disabled accounts as well as IIS service
accounts. Has anyone got any idea as to why this could be happening, or
even as to how I can monitor when and why it is happening??

I'm desparate, to the point that i'm going to rebuild all 3 servers if i
can't get to the bottom of it.
