How can I designate all mail to come 'in' from one account and 'out' from
another? That is to say, how may I assign different logins for the incoming
and outgoing mail server from a single account? Is there a way to combine
inboxes?

You will need a third party e-mail application. This is a very common
question when people start using mobile devices over wireless networks or
GPRS/CDMA because not all SMTP servers allow connection without
authentication - or sometimes you have to use a third party SMTP server.
